Care Coordination Services

For families caring for children with special healthcare needs, we partner with organizations dedicated to fostering coordinated treatment.

  • Almost Home Kids (AHK): AHK provides a bridge from hospital to home through an innovative community-based care system for children with medical complexities. 
  • Lurie Children's Health Partners Care Coordination (LCHPCC): LCHPCC offers child intensive care coordination services using a team of nurses, social workers and community health care workers to make sure children get the right care, at the right time, in the right place.
