Most people covered by Medicaid in the Chicago metro area must select (or be auto-enrolled into) a Medicaid managed care plan. If you want your child to receive services at Lurie Children's, please make sure to select a plan that includes Lurie Children's.

Determining health care benefits and coverage is very complicated. It is best to verify your coverage by reviewing your plan's benefit book or contacting your plan's customer service department for details on:

• Eligibility
• Member's financial responsibility: deductible, co-pay and co-insurance
• Plan's policy for referrals, prior-authorization or precertification
• Covered and non-covered services
• Network status of providers

Specialty Networks

Insurance companies may utilize specialty networks for some services, including but not limited to: mental health, rehabilitation (therapy), eye and/or dental care.Please be sure to call your insurance company before making an appointment to confirm that both the hospital and provider are covered by your plan.

Changes to your Insurance Coverage
