Indiana Medicaid Managed Care Plans We Accept

Determining health care benefits and coverage can be complicated. It is best to verify your coverage by reviewing your plan's benefit book or contacting your plan's customer serice department for details on:

  • Eligibility
  • Member's financial responsibility: deductible, co-pay and co-insurance
  • Plan's policy for referrals, prior-authorization or precertification
  • Covered and non-covered services
  • Network status of providers

Specialty Networks

Insurance companies may utilize specialty networks for some services, including but not limited to: mental health, rehabilitation (therapy), eye and/or dental care. Please be sure to call your insurance company before making an appointment to confirm that both the hospital and provider are covered by your plan.

Changes to your Insurance Coverage

Please notify Lurie Children's about any changes to your insurance as soon as possible.
